WHAT TO DO IN CASE OF A MEDICAL CLAIM?

In case of an emergency:

  1. Go to the hospital
  2. Present your insurance access card.
  3. The Approval will be transmitted to you through the Hospital or the delegate (if available).

Note: If you need an admission in an emergency, and you are co-insured with the National Social Security Fund (NSSF), Please make sure to get the NSSF’s late approval.

In non-emergency cases:
Provide the following documents to the Insurance office at the hospital (or directly to Fidelity), preferably 48 hours ahead of time:

  1. The official original medical report and supportive documents (showing the cause and doctor’s signature).
  2. The access card
  3. Your ID card
  4. Your prior National Social Security Fund (NSSF) approval, in case you are co-insured with NSSF.

Out-hospital (laboratory tests and X-rays…):
You can head directly to the laboratory with the following documents:

  1. The official original medical report and supportive documents (showing the Diagnosis and doctor’s signature)
  2.  The access card
  3. Your ID card

The laboratory will provide you with the Approval.

Remarks:

  1. Ask for the nurse delegate, if available, during working hours.
  2. For Assistance outside working hours call the claims center at 01/577200.
  3. All the above is subject to the policy terms, conditions and exclusions.
